Please fill out the required initial exam paperwork online through our Jane booking service (Under book an appointment). The time spent filling out the information accurately will prepare the doctor for your appointment and save more time for evaluation and treatment in office. 

We are currently contracted with American Specialty Health, Cigna, Aetna, and United Health.

We are still in the process of contracting with Pacific Source, Blue Cross/Regence, MODA, Medicaid and Medicare. Until contracts are finalized, we are providing cash time of services rates. Superbills can be prepared upon client request to submit to insurance for potential reimbursement.

Patients should wear something comfortable, or some type of athletic wear is preferred so you can move freely and perform rehabilitation more comfortably. If you have a specific extremity complaint (foot/ankle/knee/shoulder/elbow etc.) it helps to be able to have that region free of clothing or able to remove for soft tissue modalities and evaluation.

Power Performance Chiropractic is located inside 1725 SW Chandler Ave Suite 102 in the same building as Dr. Dondo Dental Excellence and MOVE Physical Therapy. Access to the parking lot is off Chandler Ave, with the entrance located around the back of the building.
